Report Fraud and/or Abuse
If you are reporting fruad or abuse of our programs and/or housing services please click HERE to print the Fraud-Abuse Form. Fill it out and fax it to 801-487-3641 or you can drop it off or mail it to our office at: 1776 S. West Temple, Salt Lake City, UT 84115.
